Additional Services: Forms, Phone and Prescription Refills
Our time and expertise are our only source of remuneration. In contrast to the industry standard of raising everyone's prices to defray the cost of the "extras", we prefer to charge only those who request them.
Prescription Refills (by fax)—$10/script
Generally your prescriptions should last until your next scheduled follow up visit. Refills should be required only in the rare event that you must cancel that appointment. Under those circumstances you will be given a small supply to hold you until you can be rescheduled.
Forms/Letters—$20-$40
(Charges will vary depending upon the complexity of the form or letter.)
